Approved Program Amendment Form

Once a program is initially proposed and approved by the Overseas Study Advisory Council (OSAC), or following program reauthorization via OSAC’s review of the post-program director’s report, there may be instances where significant changes to the program are needed or desired.  These include, but are not limited to, changes in: 

  • Change of locations within the same country/ies (for change in destination country, please complete this document and upload it to the iAbroad portal in the Program Proposal e-form).
  • Program itinerary (e.g. content, dates)
  • Faculty leader/course instructor
  • Associated course(s)
  • Support services abroad and/or logistics provider
  • Health, safety, and security situation
  • Housing arrangements
  • Participant demographics 

In such situations, program leaders must submit a program amendment form to OSAC for review and reapproval/reauthorization.  It is advised that this form be submitted at least 45 days  in advance of student recruitment and fee payment. If you aren’t sure if an anticipated change would require the submission of a program amendment form, please contact the Associate Vice President for Education Abroad at edabroad@iu.edu.

To submit a program change, please click on the link provided to access the program amendment form.  Please provide the general program information listed below and address the major changes to the program in the appropriate section(s).  Note: for ease of use, and for your own records, it is advised that answers be drafted in a word document and then cut/pasted into the form itself.

Program Adjustments: Describe in detail any major changes to the original program that apply.  If there are no changes to a specific element of the program, leave that section blank.

  • Associated course, credit (also upload revised syllabi)
  • Support services abroad and/or logistics provider
  • Health, safety, and security situation

  • Participant demographics (e.g. switch from undergraduates only to undergraduates and graduates)

Institutional Policies

Note: All educational activities abroad must follow Indiana University policies, including, but not limited to, Research with Human SubjectsPrograms Involving Children, and Snorkeling and SCUBA Diving. IU requires that sleeping arrangements provide for a separate bed for each participant.
